Health care systems around the world are suffering from a chronic disease epidemic, most frequently referred to the so-called non-communicable diseases (NCDs). As a consequence, many stakeholders in health care agree that health systems reform is critical in these days.
The symposium "An International Look at Health Care Transformation" will have an international perspective with speakers from Singapore, Cambodia, and Canada referring to health care transformation.
The Chronic Care Management Days were set-up in the context of a comprehensive research project on modeling the impacts of chronic care policy changes in Switzerland that is part of the National Research Programme "Smarter Health Care" (NRP 74).
The symposium is co-organized by Bern University of Applied Sciences, University of Bern, Duke-NUS Medical School Singapore, Mississauga Halton Local Health Integration Network Ontario (Canada), and sitem-insel.
